Students must be currently enrolled in Central Unified to participate. All Roosevelt Elementary students will receive a spirit sweater or jacket this year, no application is needed for these students!

• Only one (1) submission per student
• If you are requesting sweaters/ coats for multiple children, please fill out multiple submissions
• All information collected will be kept confidential and never shared
• Students selected, will receive a sweater or jacket before Thanksgiving break.
